External Provider Support

External providers working in schools must follow departmental policies and regulations. Access depends on school decision-making, available space, the number of providers, and the impact on administration and class teachers’ schedules.

Hawthorndene Primary School works with families and external providers to support students on site.

We follow the Department for Education’s clear decision-making processes and procedures when deciding to allow or refuse external providers to support clients/students on our school site.

The following information and links provide a clear outline of our process for seeking support from external providers at Hawthorndene Primary School.

Request and approval process

When a parent/carer wants to request an external provider to support on site they are required to:

  • complete the DfE Request for Provision of Service form and Permission to Exchange Information form available from the website and return them to school via email to Alicia.McInnes961@schools.sa.edu.au
  • provide assessment reports relating to the request, if not already received by the school
  • if you are submitting a new request, meet with Alicia to discuss the request in detail
  • if you are already receiving external provider support in school, Alicia will make contact at the beginning of the school year to ensure you would like to continue
  • provide the name, contact details and availability of the provider

 

If the request is approved, parents/carers will be notified by the school and can notify the provider.

Providers are then required to:

  • make a time to meet with school Leadership to complete DfE License Agreement and Service Schedule
  • provide all relevant documents required by DfE
  • participate in site induction
